Admission

For Admitting purposes, patients should bring the following:
Health Insurance Card(s)
Photo Identification (e.g.,  valid driver's license, passport)
Physician Orders
Prescription Medications: For inpatient and ambulatory procedures, patients should bring a list of their prescription medications.           

How to register

Outpatient Registration

Pre-Registrations are processed via the registration representative in the front lobby; after hours pre-registration are processed at the registration desk in the Emergency Department. 
Patients undergoing outpatient surgery and or endoscopy should report to the registration front desk in the front lobby. 
Please arrive at least 30 minutes before your appointment. If you have any questions, please call us and we will guide your through this process.
Outpatient Registration Hours:
Monday - Friday 6:00 a.m. - 7:00 p.m. Saturday/ Sunday 6:00 a.m. - 2:30 p.m. When Outpatient Patient Registration (front lobby) is closed, please report to the Emergency Department to register.

Inpatient Registration

All direct admission patients should report to the Emergency Department to register.
